I think what Bill is saying , is that through vaccines and healthcare people will live longer and therfore not have as many kids to compensate for shorter life.

oh my dear lord, what he literally said was that if we help with reproductive services. IE I third world countries where they have no birth control. We can slow the rate at which the population grows. Instead if it growing by 3billion he thinks we can only grow it by 1.5billion. HE NEVER MENTIONS ANYTHING ABOUT REDUCING POPULATION. I try and watch as many TED talks as I can, because they rock and I actually watched this one like 6 months ago. Love or hate MS, Gates is an amazing visionary. And I 100% agree with what he says in this talk.
